Durable Power of Attorney for Health Care:  A durable power of attorney is a legal document that enables an individual to designate another individual to act on his or her behalf, even in the event that the individual becomes disabled or incapacitated (either mentally or physically). An advance care directive, also called a healthcare directive combines a living will with the durable power of attorney, either in one document or two separate ones.
